Community Care Nurse

About the Role:
In this dynamic community nursing position, you will have the opportunity to provide high-quality care to patients in various settings, working collaboratively with Registered Nurses to support more complex patients. As a Registered Practical Nurse (RPN), you will play a vital role in helping patients remain comfortable in their homes for as long as possible.
What You'll Do:
- Participate in scheduled visits in the community to assess and address patient needs within your scope of practice.
- Provide safe and ethical nursing care, utilizing appropriate assessment tools and techniques to identify individual patient needs.
- Develop and implement care plans, continuously monitoring and adjusting as required to ensure optimal patient outcomes.
- Engage patients using a collaborative approach, building rapport and trust to promote positive health outcomes.
- Maintain and update professional patient records related to patient encounters and plans.

Requirements:
- Registration with the CNO as a Registered Practical Nurse in good standing.
- Valid driver's license and access to a vehicle.
- Experience in delivering community care is an asset.
- Reliability and flexibility.
- Willingness to travel to and from patient residences within a designated area.
- Experience working with vulnerable populations.
- Passion for making a difference in your community.
- Required on-call coverage, including after-hours to ensure patient safety and meet patient care needs.
This is a Union Position – Service Employees International Union Local 1 Canada, Windsor Essex County.

Salary: LHIN Funded Nursing Rates as per Collective Agreement
Location: County Area, Erie St. Clair District
Time Type: Full time
