About the Role

This is a challenging and rewarding opportunity for a Registered Practical Nurse to join our team at ParaMed Home Health Care. As a Community Nursing Role, you will be responsible for providing high-quality nursing care to patients in their homes, working collaboratively with our team of healthcare professionals to ensure the best possible outcomes.

Key Responsibilities

  • Participate in scheduled visits in the community to assess patient needs and provide nursing care within your scope of practice
  • Provide safe and effective nursing care, using appropriate assessment tools and techniques to identify patient needs
  • Develop and implement individualized care plans, working collaboratively with patients and their families to achieve optimal health outcomes
  • Engage patients in their care, using a collaborative approach to build rapport and trust
  • Maintain accurate and up-to-date patient records, documenting all interactions and care provided

Requirements

  • Registration with the College of Nurses of Ontario as a Registered Practical Nurse in good standing
  • Valid driver's license and access to a reliable vehicle
  • Experience in delivering community care is an asset
  • Reliability, flexibility, and a strong commitment to providing excellent patient care
