What You'll Do:

  • Participate in scheduled visits in the community to assess and address patient needs within your scope of practice.
  • Provide safe and ethical nursing care, utilizing appropriate assessment tools and techniques to identify individual patient needs.
  • Develop and implement care plans, continuously monitoring and adjusting as required to ensure optimal patient outcomes.
  • Engage patients using a collaborative approach, building rapport and trust to promote positive health outcomes.
  • Maintain and update professional patient records related to patient encounters and plans.

Requirements:

  • Registration with the CNO as a Registered Practical Nurse in good standing.
  • Valid driver's license and access to a vehicle.
  • Experience in delivering community care is an asset.
  • Reliability and flexibility.
  • Willingness to travel to and from patient residences within a designated area.
  • Experience working with vulnerable populations.
  • Passion for making a difference in your community.
  • Required on-call coverage, including after-hours to ensure patient safety and meet patient care needs.
