About the Role:

We are seeking a skilled Registered Practical Nurse to join our team in a community nursing role. As a RPN, you will provide high-quality care to patients in their homes, working collaboratively with RNs to support complex patients. You will have the opportunity to build long-term relationships with patients and their families, making a meaningful difference in their lives.

Key Responsibilities:

  • Participate in scheduled visits to assess patient needs and provide care within your scope of practice
  • Provide safe and ethical nursing care, using appropriate assessment tools and techniques
  • Develop and implement care plans, monitoring patient progress and adjusting as needed
  • Engage patients and their families in their care, promoting independence and empowerment
  • Maintain accurate and up-to-date patient records

Requirements:

  • Registration with the CNO as a Registered Practical Nurse in good standing
  • Valid driver's license and access to a vehicle
  • Experience in community care is an asset
  • Reliability, flexibility, and a passion for making a difference in your community
