The Clinical Dietitian ensures that nutrition care plans are outcome-focused, evidence-based, and optimized for both clinical and fiscal outcomes, emphasizing health promotion and disease prevention.
In the ambulatory setting, the Clinical Dietitian provides patients with effective, evidence-based strategies to self-manage their disease, reducing hospital admission rates. By ensuring optimal nutrition status for patients requiring hospitalization, the Clinical Dietitian enhances medical management, leading to improved clinical outcomes such as decreased infection rates, shorter hospital stays, and reduced readmission rates.
As a member of a primary healthcare (PHC) Diabetes Centre team, the PHC Dietitian provides a range of PHC services. The Dietitian with a focus on Diabetes care uses critical thinking and clinical judgment to coordinate, facilitate, and improve the management of patients with Diabetes and co-occurring chronic conditions within an integrated primary healthcare model.
About YouWe are seeking a highly skilled and experienced Clinical Dietitian to join our team. The ideal candidate will possess:
- Bachelor of Science degree in Nutrition/Dietetics
- Completion of an approved Dietetic internship
- Licensure with the Nova Scotia College of Dietitians and Nutritionists
- Designation as a Certified Diabetes Educator (CDE) or willingness to obtain within 2 years
- Experience with group education and ability to work effectively in a team environment
- Strong communication, conflict resolution, and collaboration skills
- Leadership, planning, organization, and time management skills
- Commitment to promoting a culture that supports safety, ethical practices, and organizational health, as well as diversity, equity, and inclusion
This is a permanent, full-time position with a 75-hour bi-weekly schedule. Shifts may include days, evenings, and weekends, with occasional travel between sites required.
