Job Description


We are seeking a highly skilled Geriatric Care Specialist to join our team. As a key member of our Mobile Support Team, you will provide expert care and assistance to residents of long-term care homes with complex needs. Your primary responsibility will be to work collaboratively with Registered Nurses and Registered Practical Nurses to enhance the quality of life for these individuals.



Responsibilities



  • Provide personal care and support to residents with activities of daily living, including bathing, dressing, and grooming.
  • Work closely with healthcare professionals to develop and implement individualized care plans that meet the unique needs of each resident.
  • Conduct crisis-intervention services and provide ongoing training for residents, families, and clinicians.
  • Collaborate with inter-professional care teams to ensure seamless transitions between healthcare providers.
  • Document resident progress and outcomes accurately and thoroughly.


Requirements


To be successful in this role, you must possess:



  • A Personal Support Worker (PSW) certification as per the Long Term Care Homes Act and Regulations.
  • A current Certificate of Basic Cardiac Life Support (BCLS) or willingness to obtain within three months of employment.
  • A valid driver's license and access to a reliable vehicle at all times.
  • Familiarity with the Long Term Care Homes Act and Regulations.
  • Sound understanding of and commitment to the principles of the Behavioral Supports Ontario Project and the services re-design plan of the Central LHIN.
  • Knowledge of GPA, U-First, or willingness to receive such training.
  • Ability to implement best practice in dementia, delirium, mental health issues, and their effect on persons, families, and caregivers.
  • Effectiveness working with minimal supervision.
  • Ability to meet the physical demands of the position.
  • Proven attendance record.
  • Demonstrates understanding and complies with patient and staff safety policies and procedures that foster a safe and inclusive environment for all.
  • Ability to perform the essential duties of the job, including hands-on patient support and data collection.


Preferred Qualifications


While not required, having one year of experience working with geriatric clients in long-term care homes would be an asset. Additionally, proficiency in a second language may be beneficial in providing culturally sensitive care to our diverse patient population.



Compensation and Benefits


The estimated salary for this role is $55,000-$65,000 per annum, depending on experience. We offer a comprehensive benefits package, including health, dental, and vision coverage, as well as opportunities for professional development and growth.
