About the Role:

VON Canada (Victorian Order of Nurses) is seeking a Licensed Practical Nurse to join our team in a Home Care and Visiting Care Agency setting. As a Licensed Practical Nurse, you will provide high-quality patient care and support to individuals in their homes, promoting independence and dignity.

Responsibilities:

  • Provide nursing care and support to patients in their homes, including wound care, medication management, and vital sign monitoring.
  • Collaborate with healthcare teams to develop and implement individualized care plans.
  • Conduct home visits to assess patient needs and provide education on self-care and disease management.
  • Document patient information and progress in a timely and accurate manner.
  • Participate in ongoing education and training to maintain current knowledge and skills.

Requirements:

  • Licensure as a Licensed Practical Nurse in the province of employment.
  • Current CPR certification.
  • Valid driver's license and reliable transportation.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
