About the Role
The Licensed Practical Nurse is a vital member of our care team, providing exceptional support and care to our residents. As a key contributor to our interdisciplinary team, you will be responsible for delivering high-quality care that prioritizes the unique needs and preferences of each resident.
Key Responsibilities
- Resident-Centered Care: Provide comprehensive nursing assessments, develop and implement individualized care plans, and collaborate with the interdisciplinary team to ensure seamless care delivery.
- Documentation: Accurately and thoroughly document resident care, including assessments, interventions, and outcomes, in accordance with Bethany Care Society policies and procedures.
- Safety: Contribute to a safe environment for residents, staff, and self by adhering to safety guidelines and best practices at all times.
- Pro-Active Learner: Demonstrate a commitment to ongoing learning and professional development, attending education opportunities and sharing knowledge with the team to enhance resident care.
- Team Work: Foster a culture of collaboration and respect among team members, promoting open communication and a shared commitment to resident-centered care.
- Leadership: Provide direction and supervision to Health Care Aides, and assume the 'In Charge' role as needed, to ensure seamless care delivery.
Requirements
- Graduate of a recognized Licensed Practical Nursing program
- Previous LPN experience is preferred
- Active Registration and good standing with CLPNA
- Able to work effectively in a physically and mentally demanding environment
- Demonstrates ability to understand, speak, read, and write effectively in English
- Excellent observation, communication, and critical thinking skills
- Current CPR certification
- Dementia Care Certificate and experience preferred
- Knowledge of RAI/MDS 2.0 tool, computer skills, and electronic documentation is preferred
