Continuing Care Coordinator

The Care Coordinator plays a vital role in the assessment, care planning, and authorization of services for clients referred to Continuing Care. This position requires strong assessment and interviewing skills, as well as the ability to work independently and collaboratively in a busy multidisciplinary environment.
The Care Coordinator is responsible for identifying clients' goals, unmet needs, and resource requirements, and formulating a plan to meet those goals. They work with interprofessional teams to facilitate hospital discharge and return clients to the community, while also authorizing services and making referrals to other professional and volunteer resources.
This role requires a strong understanding of community agencies and resources, as well as excellent verbal and written communication skills. The Care Coordinator must be able to work effectively with clients, family members, caregivers, and healthcare teams to achieve the best possible outcomes.
About YouWe are seeking a highly skilled and compassionate Care Coordinator to join our team. The ideal candidate will have a Bachelor's degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies, and current registration with the applicable Nova Scotia Professional Regulatory body.
The successful candidate will have a minimum of three years' experience in a community or long-term care setting, with demonstrated knowledge and experience in dedicated case management or discharge planning. They will also possess excellent assessment, interviewing, and negotiation skills, as well as the ability to work independently and collaboratively in a busy multidisciplinary environment.

Hours and CompensationThis is a permanent full-time position, with a 70-hour bi-weekly schedule. Shifts may include days, evenings, weekends, and holidays. The hourly rate is $41.06 - $46.75, and successful candidates may be eligible for our benefits package, which includes health, dental, travel, long-term disability, and life insurance coverage, as well as a defined benefit pension plan.
