About the Opportunity

The Continuing Care Coordinator is responsible for assessing, planning, and authorizing services for clients referred to Continuing Care. This role involves working with interprofessional teams to facilitate hospital discharge and support community-dwelling clients. The Care Coordinator must be able to identify client goals, unmet needs, and resource requirements, and develop a plan to meet these needs.

The ideal candidate will have a Bachelor's Degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies, and current registration with the applicable Nova Scotia Professional Regulatory body. A minimum of three years' experience in a community or long-term care setting is required, as well as knowledge of community agencies and resources. Excellent assessment, interviewing, and negotiation skills are essential, as well as the ability to work independently and co-operatively in a busy multidisciplinary situation.

About You
  • Must have a Bachelor's Degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies
  • Current and ongoing registration with the applicable Nova Scotia Professional Regulatory body
  • Minimum three years' experience in a community or long-term care setting
  • Demonstrated knowledge and/or experience in dedicated case management or discharge planning
  • Competent with computer use, keyboarding, and able to conduct assessments using a laptop
  • Knowledge of community agencies and resources
  • Excellent assessment, interviewing, and negotiation skills
  • Excellent verbal and written communication skills
  • Ability to work independently and co-operatively in a busy multidisciplinary situation
  • Ability to multi-task, prioritize workload, and manage time efficiently
  • Valid driver's license and access to a reliable vehicle
  • Competencies in other languages preferred; French an asset
Hours
  • Permanent Full Time; 70 Hours Bi-weekly
  • Shifts may include days, evenings, weekends, and holidays
Compensation and Benefits

$41.06 - $46.75 Hourly
