About the Role
The Nova Scotia Health Authority is seeking a Patient Care Specialist to join our team. As a Patient Care Specialist, you will play a critical role in providing care coordination and support services to patients in the community. You will work collaboratively with interprofessional teams to assess patient needs, develop and implement care plans, and provide ongoing case management. Responsibilities
- Assess patient needs and develop individualized care plans
- Provide education and support to patients and their families
- Collaborate with healthcare teams to coordinate care and services
- Monitor patient progress and adjust care plans as needed

Requirements
- Bachelor's degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies
- Current registration with the applicable Nova Scotia Professional Regulatory body
- Minimum three years' experience in a community or long-term care setting
- Demonstrated knowledge and/or experience in case management or discharge planning
