Job Summary

We are seeking a dynamic and experienced Registered Nurse (RN) to join our team as a Community Care Nurse Lead. This role involves working closely with interdisciplinary teams to provide comprehensive care to clients in their homes and communities.

The successful candidate will have a strong clinical background, excellent assessment skills, and the ability to lead and collaborate with teams to achieve outstanding client outcomes.

Key Responsibilities
  • Provide comprehensive care to clients in their homes and communities, including assessments, planning, implementation, and evaluation of care plans.
  • Lead and collaborate with interdisciplinary teams to ensure seamless care transitions and optimal client outcomes.
  • Develop and maintain strong relationships with clients, families, and caregivers to ensure personalized care that meets their unique needs and goals.
  • Communicate effectively with teams and stakeholders to ensure clear understanding and alignment around client care plans and goals.
Requirements
  • Current registration with the College of Nurses of Ontario (CNO)
  • Minimum two years of nursing experience, preferably in home/community care or similar settings
  • Strong clinical leadership skills, including the ability to lead and motivate teams
  • Excellent communication, interpersonal, and problem-solving skills
  • Able to work independently and collaboratively within a team environment

  • Competitive salary range: $83,500 - $103,000 per year, depending on experience
