Job Overview

We are seeking a dynamic and compassionate Registered Practical Nurse (RPN) to join our team as a Care Coordinator for Community Health. This role involves working collaboratively with interdisciplinary teams to provide comprehensive care and support to clients in their homes and communities.

Key Responsibilities:
  • Provide client-centered care and support, prioritizing their unique needs and goals.
  • Collaborate with healthcare professionals to develop and implement individualized care plans.
  • Conduct thorough assessments and evaluations to identify client needs and opportunities for growth.
  • Lead interdisciplinary team meetings to discuss client progress and address any concerns or challenges.
  • Foster strong relationships with clients, families, and caregivers, ensuring open communication and empathy.
Requirements:
  • Current registration with the College of Nurses of Ontario.
  • Bachelor's degree in nursing or equivalent experience.
  • Minimum two years of nursing experience, preferably in home or community care.
  • Demonstrated clinical leadership skills and ability to work independently.
  • Excellent assessment and communication skills, with a strong understanding of community health practices.
Salary Information:

The estimated salary for this position is $85,000 - $95,000 per year, depending on experience and qualifications.
