Job OverviewWe are seeking a dynamic and compassionate Registered Practical Nurse (RPN) to join our team as a Care Coordinator for Community Health. This role involves working collaboratively with interdisciplinary teams to provide comprehensive care and support to clients in their homes and communities.
Key Responsibilities:- Provide client-centered care and support, prioritizing their unique needs and goals.
- Collaborate with healthcare professionals to develop and implement individualized care plans.
- Conduct thorough assessments and evaluations to identify client needs and opportunities for growth.
- Lead interdisciplinary team meetings to discuss client progress and address any concerns or challenges.
- Foster strong relationships with clients, families, and caregivers, ensuring open communication and empathy.
- Current registration with the College of Nurses of Ontario.
- Bachelor's degree in nursing or equivalent experience.
- Minimum two years of nursing experience, preferably in home or community care.
- Demonstrated clinical leadership skills and ability to work independently.
- Excellent assessment and communication skills, with a strong understanding of community health practices.
The estimated salary for this position is $85,000 - $95,000 per year, depending on experience and qualifications.
