Nurse Care Specialist

We are seeking a dedicated and passionate Registered Nurse to join our Transitions team in Canada. This is an exciting opportunity to be at the forefront of healthcare innovation, providing comprehensive care in an interdisciplinary team environment.
Responsibilities:- Provide continuous and comprehensive support to your team in delivering care for clients
- Lead the care planning process, working with other disciplines to complete and implement client care plans
- Facilitate regular interdisciplinary team communication, including huddles, rounds, and status updates
- Support clients through care transitions by collaborating and connecting them with community support services
- Registered in good standing with the College of Nurses of Ontario
- Minimum of two years of nursing experience
- Experience in home/community care, acute care, chronic disease management, health promotion, palliative care, or mental health
- Ability to work independently in an autonomous and self-directed manner within RN scope of practice
- Competitive salary: $70,000 - $90,000 per year, based on experience
