About the Role:

  • We are seeking a dynamic and passionate Clinical Care Professional to join our team. The ideal candidate will have a strong clinical background, excellent assessment skills, and a proven ability to work independently and in a team environment.\
  • The successful candidate will provide comprehensive care in an interdisciplinary team setting, working closely with other healthcare professionals to deliver high-quality patient outcomes.\
  • You will be responsible for conducting client assessments, developing care plans, and leading discharge and transition care planning discussions.\
  • Additionally, you will facilitate regular interdisciplinary team communication, support clients through care transitions, and collaborate with community support services to optimize client outcomes.\
  • This role requires a strong understanding of community nursing principles, excellent interpersonal and communication skills, and a demonstrated ability to work effectively in a fast-paced environment.\
  • The successful candidate will be registered in good standing with the College of Nurses of Ontario, hold a Bachelor's Degree in Nursing (or equivalent), and have a minimum of two years of nursing experience.\
  • Prior experience in home/community care, acute care, chronic disease management, health promotion, palliative care, or mental health is highly desirable.\
  • Membership in RNAO and Canadian Nurses Association (CNA) community nursing certification are considered assets.\
  • A valid driver's license and vehicle are required for this role, as well as a Vulnerable Sector Check.\
