We are seeking a highly skilled and dedicated Community Health Specialist to join our team at SE Health.
About the RoleThis is an exciting opportunity for a Registered Practical Nurse (RPN) with a passion for community care to provide high-quality patient-centered care in a dynamic and supportive environment. As a Community Health Specialist, you will be part of a collaborative team that delivers comprehensive care to clients in their homes and communities.
You will work closely with interdisciplinary teams to assess client needs, develop and implement personalized care plans, and provide ongoing support to ensure optimal health outcomes. Your expertise in nursing and ability to communicate effectively will make a significant difference in the lives of our clients.

What You Will Be Doing- Assessing client needs and developing personalized care plans
- Providing high-quality patient-centered care in home and community settings
- Working collaboratively with interdisciplinary teams to achieve optimal health outcomes
- Communicating effectively with clients, families, and healthcare professionals
- Serving as a primary contact for clients and their families
- Registered in good standing with the College of Nurses of Ontario
- Bachelor's Degree in nursing preferred
- Canadian Nurses Association (CNA) community nursing certification is an asset
- Minimum of two years of nursing experience
- Experience in one of the following areas: home/community care, acute care, chronic disease management, health promotion, palliative care, and mental health
The estimated salary for this role is $65,000 to $85,000 per year, depending on experience. Additionally, you will receive a comprehensive benefits package, including medical, dental, and vision coverage, as well as a pension plan and opportunities for professional development.
