About the Role

The Canadian Mental Health Association Toronto Branch is seeking a highly skilled and compassionate Case Manager to join our team. As a key member of our Transitional Rehabilitation Housing Program 2 (TRHP 2), you will play a vital role in supporting individuals with complex mental health needs.

Key Responsibilities
  • Develop and implement individualized service plans to promote recovery and community reintegration
  • Provide on-site and community-based services, including support with daily living skills, structured day activities, and relapse prevention
  • Collaborate with interdisciplinary teams to ensure comprehensive care and support
  • Conduct risk assessments and develop strategies to mitigate risks
  • Build and maintain trusting relationships with clients, colleagues, and community partners
Requirements
  • Undergraduate degree or diploma in social services or a related field
  • Minimum 3 years' experience in direct service delivery to adults with complex mental health issues
  • Strong knowledge of forensic mental health, the criminal justice system, and psychosocial rehabilitation
  • Excellent assessment, service planning, and crisis prevention/intervention skills
  • Ability to work with diverse communities and demonstrate an anti-oppression approach to practice
