Job Title: Case Manager

WoodGreen is seeking a skilled Case Manager to join our team in providing comprehensive support services to individuals in our Supportive Housing Program.

Job Summary:

The Case Manager will work closely with clients to assess their needs, develop personalized case plans, and provide ongoing support to help them achieve their goals. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a team environment.

Key Responsibilities:
  • Conduct comprehensive assessments of clients' needs and develop individualized case plans
  • Provide ongoing support and guidance to clients to help them achieve their goals
  • Collaborate with other service providers to ensure a continuum of care
  • Monitor clients' progress and make adjustments to case plans as needed
  • Maintain accurate records and statistics
Requirements:
  • Bachelor's degree in Social Work or a related field
  • 3+ years of experience in mental health and housing support
  • Strong communication and interpersonal skills
  • Ability to work effectively in a team environment
