Job Summary

WoodGreen is seeking a skilled Case Manager to join our Supportive Housing Program. As a Case Manager, you will provide comprehensive case management services to individuals in the program, including assessment, personal goal setting, and referrals to community resources. You will work closely with staff and clients to develop and implement individualized plans, and provide ongoing support and guidance to help clients achieve their goals.

Key Responsibilities
  • Provide case management services to individuals in the program, including assessment, personal goal setting, and referrals to community resources.
  • Develop and implement individualized plans with clients, and provide ongoing support and guidance to help them achieve their goals.
  • Work closely with staff and clients to identify and address mental health issues, and connect clients with primary care and other needed resources.
  • Assist tenants with daily living tasks, such as grocery shopping and meal preparation, and provide support with life skills development.
  • Monitor the housing challenges and ability of each resident to adhere to the policies and protocols, and develop a team strategy for intervention when problems occur.
  • Complete statistics and reports, and maintain all records regarding program activity enrollment.

Requirements
  • Bachelor's degree in Social Work or a related discipline.
  • 3+ years of experience in mental health and harm reduction, housing support, and working with individuals who have experienced homelessness.
  • Ability to work effectively in a team and independently, and to exercise good judgment, flexibility, and creativity in a fast-paced environment.
  • Strong communication and interpersonal skills, and ability to develop and maintain positive relationships with clients, staff, and community partners.
