Job Title: Case Manager

S.U.C.C.E.S.S. is seeking a skilled Case Manager to join our team. As a Case Manager, you will be responsible for providing support and guidance to clients, ensuring they receive the necessary resources and services to achieve their goals.

Key Responsibilities:
  • Provide case management services to clients, including assessment, planning, and coordination of services.
  • Develop and implement individualized plans to meet the unique needs of each client.
  • Collaborate with internal and external stakeholders to ensure seamless service delivery.
  • Monitor and evaluate client progress, making adjustments to plans as needed.
  • Maintain accurate and up-to-date records of client interactions and progress.
Requirements:
  • Bachelor's degree in a related field.
  • 1 year to less than 2 years of experience in case management or a related field.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong problem-solving and critical thinking skills.
