Job Summary

We are seeking a highly skilled and compassionate Social Service Worker to join our team at Extendicare Canada Inc. As a Social Service Worker, you will play a vital role in providing exceptional care and support to our residents, ensuring their physical, emotional, and social needs are met.

Key Responsibilities
  • Participate in resident admission process, including tours, admission, transfers, discharge, and occupancy tracking.
  • Act as liaison with families and advocate for residents to ensure they are allowed and encouraged to exercise their rights.
  • Participate in the interdisciplinary team process of resident assessment, care planning, and evaluation to ensure services are planned according to individual resident and family needs.
  • Coordinate services of Psychogeriatric Consultant in support of staff caring for residents.
  • Adhere to established department plans, policies, procedures, and quality assurance program to meet all regulatory and Extendicare standards.
  • Provide in-service programs for home staff, residents, and families. Encourage and support professional growth and development of staff.
  • Liaise with educational institutions and provide supervision to social service work students during field practicum. Promote other quality initiatives and programs designed to enhance resident care.
  • Maintain all required records, reports, statistics, and documentation for both residents and department in accordance with policies, procedures, and legislative requirements.
  • Participate on committees related to resident care and operation of the home as required.
  • Liaise and consult with community partners, company resources, and professionals in relation to resident care needs and department activities.
  • Acknowledge and request referral to a Social Worker for areas beyond scope of practice.
  • Assist with recruitment, screening, orienting, and recognizing volunteers.
Requirements
  • Registration with Ontario College of Social Workers is mandatory.
  • Previous experience in a long-term care setting is preferred.
  • Excellent communicator, well-organized team player with solid clinical skills in assessments and care planning, supportive therapy, crisis intervention, and grief counseling is preferred.
  • Expertise in working with geriatric populations with specialization in Alzheimer's disease and other dementias, depression, responsive behaviors, and other psychogeriatric issues is preferred.
  • Knowledge of applicable Ministry of Health and Long-Term Care legislation and standards is preferred.
  • Knowledge of occupational health and safety practices, principles, and legislation is preferred.
  • Computer literate with understanding of RAI-MDS and PointClickCare software system is preferred.
