About the Role

We are seeking a dedicated and compassionate Social Worker to join our team at Spark Lifecare. As a Social Worker, you will work with adults, children, and families to provide psycho-social rehabilitation and assessment services. Your expertise in complex mental health, case management, neurorehabilitation, and chronic pain will be invaluable in supporting our clients to achieve their goals.

Key Responsibilities
  • Provide psycho-social rehabilitation and assessment services to adults, children, and families
  • Develop and implement individualized care plans to support clients' recovery goals
  • Collaborate with multidisciplinary teams to ensure comprehensive care
  • Conduct assessments and evaluations to identify clients' needs and develop strategies for support
  • Provide education and support to clients and their families on mental health, wellness, and rehabilitation
Requirements
  • Registration in good standing with the Ontario College of Social Workers and Social Service Workers
  • Masters Degree in Social Work is an asset
  • Experience working with the MVA (Motor Vehicle Accident) system will be an asset
  • Registration with the Order of Social Workers and Marriage and Family Therapists of Quebec is an asset
  • Professional Liability Insurance
  • Valid drivers license and access to a vehicle
  • Strong verbal and written communication skills
  • Strong interpersonal skills and proven ability to work independently as well as part of a multidisciplinary rehabilitation team
  • Computer skills (i.e. MS Word, Outlook)
  • Bilingualism in a second language is a bonus, specifically French, Arabic, & Persian
