Job Summary

We are seeking a highly skilled and dedicated Registered Nurse (Nurse Practitioner) to join our Primary Health Care team in Weyburn, Saskatchewan. As a key member of our team, you will provide high-quality primary health care services to assigned populations, including urban and rural communities.

Key Responsibilities
  • Provide comprehensive primary health care services, including health assessments, health promotion, and disease prevention and treatment.
  • Collaborate with Primary Health Care Teams, physicians, and other healthcare providers to ensure seamless care delivery.
  • Develop and deliver formal education programs to promote health and wellness.
  • Participate in Lean, patient safety, community development, clinical practice redesign, quality improvement, and evaluation initiatives.
Requirements
  • Master of Nursing - Nurse Practitioner (MN-NP) degree.
  • Three (3) years nursing experience, with experience in community needs assessment/development/health promotion and Primary Health Care settings preferred.
  • Valid Class 5 driver's license and ability to competently use computers and software applications.
  • Ability to work independently and as a member of a multi-disciplinary team, with strong interpersonal, leadership, and problem-solving skills.
