Job Description

As a Clinical Care Coordinator, you will play a vital role in ensuring seamless delivery of care to our clients. Your responsibilities will include developing individualized care plans, managing chronic and communicable disease tracking and treatment, administering medications and treatments, and educating clients on healthcare needs and self-care techniques.

Required Skills and Qualifications

To be successful in this role, you must have a BScN graduate degree from a recognized nursing program, current Occupational First Aid Level 1 and CPR C certification, and at least 1 year of clinical experience in a healthcare setting. Strong communication and collaboration skills are essential, as well as a commitment to providing compassionate and patient-centered care.

Work Environment

You will work a standard 8-hour shift, Monday to Friday, with the ability to commute or relocate to Vancouver, BC. In accordance with provincial health orders, all employees must be vaccinated against Covid-19.
