Palliative Care Specialist

Job Summary
The Palliative Care Specialist will provide comprehensive care to patients with complex medical needs, utilizing a holistic approach that emphasizes partnership and collaboration with individuals, families, and communities.
Key Responsibilities
- Conduct thorough health assessments and develop individualized plans of care
- Manage symptoms and provide palliative care in both inpatient and outpatient settings
- Collaborate with physicians and interdisciplinary teams to ensure comprehensive care
- Provide education and support to patients and families
- Develop and implement policies and procedures to promote quality care
Requirements
- Current registration in the Extended Class (NP-Adult) with the College of Nurses of Ontario
- Completion of a Master Degree in Nursing
- Minimum of 3 years nursing experience in palliative care
- Exceptional knowledge in palliative care and pain & symptom management
- Formal training and education in Palliative Care with minimum of LEAP Course completion
Preferred Qualifications
- Eligibility for cross-appointment to an academic institution
- Membership in specialty related organizations NPAO, RNAO
- Innovative, creative and professional role model
- Demonstrated ability to prioritize/organize multiple demands
- Ability to work autonomously
