Job Title: Clinical Lead Nurse

Job Overview:
We are looking for a skilled and committed Clinical Lead Nurse to become a vital part of our healthcare team at Madison Village Long Term Care. The Clinical Lead Nurse will play a crucial role in managing and coordinating the delivery of patient care, offering clinical support to nursing personnel, and ensuring compliance with healthcare standards and protocols.

This position operates under the guidance of the Clinical Manager (CM) and supervises nursing care practices for residents, in alignment with required medical care and established nursing care plans. Responsibilities include assessment, data collection, and monitoring of mandatory programs and incidents related to resident care and safety.

The Clinical Lead Nurse will collaborate effectively with other team members, providing assistance and coverage during absences. This role includes supervising unit activities and directing RPNs and direct care staff on the unit.

Key Responsibilities:

  1. Receive shift reports from the Charge Nurse upon arrival and assume primary supervision of the assigned unit/area.
  2. Address any concerns from the previous 24 hours, including pain management, falls, infections, wound care, behavioral issues, resident transfers to and from hospitals, pending lab reports, and significant changes in residents' conditions.
  3. Prepare for and facilitate physician rounds, ensuring that orders are verified, documentation is organized for the upcoming week, lab paperwork is processed, and relevant parties are notified.
  4. Review residents' charts and ensure that necessary actions are taken promptly. Inform the Charge Nurse/Clinical Manager, and when appropriate, the physician and responsible substitute decision maker (SDM) of any changes in residents' conditions.
  5. Ensure that care plans are developed, updated as necessary, and reviewed quarterly according to the established schedule.
  6. Supervise and guide RPNs and PSWs to ensure that resident care aligns with care plans and the policies and procedures of the facility.

Qualifications:
- Valid RN/RPN license
- Current registration with the College of Nurses of Ontario
- Experience in Long-Term Care
- Strong understanding of geriatrics, LTC, and the FLTCA, 2021, along with associated regulations.
- Ability to thrive in a fast-paced environment
- Excellent communication and interpersonal skills
- Compassionate approach to patient care
