Job Title: Clinical Care Coordinator

We are seeking a highly skilled and compassionate Clinical Care Coordinator to join our team at Home and Community Care Support Services. As a Clinical Care Coordinator, you will play a vital role in providing high-quality care and support to our patients, ensuring they receive the best possible outcomes.

Key Responsibilities:
  • Develop and implement comprehensive care plans that meet the unique needs of each patient.
  • Collaborate with healthcare professionals, patients, and families to ensure seamless care transitions and optimal health outcomes.
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective care strategies.
  • Provide education and support to patients and families on disease management, self-care, and health promotion.
  • Facilitate communication between healthcare providers, patients, and families to ensure timely and effective care.
Requirements:
  • Bachelor's degree in Nursing or related field.
  • Minimum 2-3 years of experience in a clinical setting, preferably in community health or gerontology.
  • Fluent in verbal and written French.
  • Strong knowledge of the Ontario healthcare system and community resources.
  • Excellent communication, interpersonal, and problem-solving skills.
