Pediatric Community Care Nurse

About the Role:
As a Pediatric Community Care Nurse with ParaMed Home Health Care, you will have the opportunity to make a meaningful difference in the lives of patients in your community. This role involves providing scheduled visits to patients in various settings, working with patients of all ages and complexity of care needs, and playing a key role in helping patients remain comfortable in their homes for as long as possible.
Key Responsibilities:
- Using appropriate assessment tools and techniques to assess patients' individualized needs
- Executing and continuously monitoring care plans
- Engaging patients using a collaborative approach by building rapport and trust
- Maintaining and updating professional patient records related to patient encounters and plans
- Available for overnight shifts with pediatric clients (11pm - 7pm)
Requirements:
- Registration with the CNO as a RPN or RN in good standing
- Valid driver's license and access to a vehicle is required
- Experience in delivering community care is an asset
- Reliability and flexibility
- Willingness to travel to and from patient residences within a designated area
- Passion for making a difference in your community
- Required on-call coverage, including after hours to ensure patient safety and to meet patient care needs
