We are seeking a dynamic and passionate Registered Practical Nurse (RPN) to join our interdisciplinary care team in home and community care. As a key member of the team, you will be responsible for providing comprehensive care to clients in their own homes, promoting health and well-being through client-centered approaches.
Key Responsibilities:- Conduct thorough assessments of clients' physical, cognitive, social, emotional, and spiritual domains to develop holistic care plans
- Lead interdisciplinary care planning discussions with the care team to ensure seamless transitions and optimal outcomes
- Provide continuous support to clients and families, anticipating and addressing health issues through effective health promotion strategies
- Collaborate with community support services to ensure smooth care transitions and optimize client outcomes
- Bachelor's Degree in nursing preferred
- Canadian Nurses Association (CNA) community nursing certification is an asset
- Minimum of two years of nursing experience, preferably in home/community care, acute care, chronic disease management, or health promotion
- Diverse nursing experiences with different client groups are highly valued
- Competitive salary: $70,000 - $90,000 per year
