Salary & BenefitsWe offer a competitive rate paid per client visit, rather than care code or care time. Our Total Rewards package includes a salary of approximately $90,000 per year, group benefits, RRSP pension, and exclusive perks/discounts available only to SE Health staff.
Job DescriptionThis Nursing position is part of the Community Commitment Program for Nurses (CCPN) which provides a $25,000 incentive to eligible nurses in exchange for a 24-month commitment to an eligible employer. As a Registered Nurse or Practical Nurse, you'll work in collaboration with other care team members and support functions to deliver high-quality care to our clients.
- You will be responsible for completing comprehensive client assessments, developing care plans, implementing and evaluating care based on best evidence/practices and on client's unique needs.
- When designated as a primary provider for a client caseload, you will be the point of care lead accountable for their care while working in collaboration with other care team members and the support functions.
- You will support the client through care transitions by collaborating and connecting the client with community support services in their local community.
To succeed in this role, you should have:
- A valid driver's license and vehicle (travel required)
- Availability to work days, evenings, nights, and every other weekend
- Enthusiasm and love of community care and passion for evidence-informed practice & excellence in care
