We are seeking a highly skilled Registered Practical Nurse to join our team as a Transformative Community Care Clinician. In this role, you will be responsible for providing comprehensive care to clients in their homes and communities, working collaboratively with an interdisciplinary team to develop and implement individualized care plans.

Responsibilities:

  • ">
  • Providing comprehensive care to clients in their homes and communities">
  • Working collaboratively with an interdisciplinary team to develop and implement individualized care plans">
  • Conducting assessments and developing care plans based on best evidence and client needs">
  • Facilitating regular communication between healthcare providers and family members">
  • Supporting clients through transitions and connecting them with community resources">
  • Developing strong relationships with clients, families, and caregivers">

Requirements:

  • ">
  • Registered in good standing with the College of Nurses of Ontario">
  • Bachelor's degree in nursing preferred">
  • Canadian Nurses Association (CNA) community nursing certification is an asset">
  • Minimum of two years of nursing experience">
  • Experience in home/community care, acute care, chronic disease management, health promotion, palliative care, and mental health">
  • Demonstrated clinical leadership experience">
  • Excellent assessment skills and a strong clinical background">
  • Ability to work independently and collaboratively as part of a team">
  • Vulnerable sector check (current)">
  • A vehicle and valid driver's license and the ability to travel">
  • Membership in RNAO strongly preferred">

Compensation:

Estimated salary range: $80,000 - $110,000 per year, depending on experience
