Nurse Care Coordinator

We are seeking a dynamic and passionate Registered Nurse to join our Transitions team. As a key member of our care coordination team, you will play a vital role in providing comprehensive care to clients in their homes and communities.
The successful candidate will have a strong clinical background, excellent assessment skills, and a proven ability to work independently and as part of a collaborative team. You will be responsible for conducting client assessments, developing individualized care plans, and coordinating with healthcare professionals to ensure seamless transitions of care.

Responsibilities- Conduct comprehensive client assessments to identify needs and develop individualized care plans
- Coordinate with healthcare professionals to ensure seamless transitions of care
- Provide ongoing support and education to clients and their families
- Maintain accurate records and reports of client interactions and progress
Please note that this position requires a valid driver's license and access to a reliable vehicle, as travel to various locations may be required.
Requirements- Registered in good standing with the College of Nurses of Ontario
- Minimum of two years of nursing experience
- Experience in home and community care or a related field
- Strong clinical leadership skills and ability to work independently
- Excellent communication and interpersonal skills
A competitive salary and benefits package are offered for this full-time position. The estimated salary range for this position is $85,000 - $110,000 per year, depending on experience and qualifications.
