Job Description

As a Registered Nurse with SE Health, you'll have the opportunity to work in a dynamic and challenging environment that requires strong clinical skills, excellent communication, and a patient-centered approach. Your primary responsibilities will include:

  • Providing comprehensive care to clients in an interdisciplinary team setting
  • Serving as a primary contact for clients when designated as the primary care provider/clinician
  • Collaborating with colleagues to develop and implement individualized care plans
  • Facilitating regular team meetings and leading discharge planning discussions
  • Supporting clients through transitions by connecting them with community resources
Requirements

To succeed in this role, you'll need:

  • A degree in nursing from an accredited institution
  • Current registration with the College of Nurses of Ontario
  • A minimum of two years of experience in a related field, preferably in home/community care or acute care
  • Strong assessment and leadership skills, with the ability to work independently and collaboratively
  • An enthusiasm for community-based care and a commitment to ongoing learning and professional development
Compensation and Benefits

We offer a competitive salary range of $85,000 - $110,000 per year, depending on experience, as well as a comprehensive benefits package, including group health insurance, retirement savings plan, and opportunities for professional growth and development.
