The Role

As a Wellness Navigator at Nova Scotia Health, you'll demonstrate flexibility and innovation in supporting citizens accessing services, programs, and resources that promote healthy behaviors.

You'll provide guidance and support for clients, including information about appropriate supports and programs meeting their health and wellness needs and social supports assisting with daily living.

Key Responsibilities

  • Develop and facilitate emotional wellness programming using a person-centered and family-focused approach.
  • Facilitate other health and wellness programs to promote active involvement of participants and families in establishing person-centered goals.
  • Collaborate with the Community Health Team staff, family physicians, other healthcare providers, and community groups to maintain awareness of existing supports within the community.
  • Work collaboratively with the team to develop, implement, and evaluate the community-based wellness navigation model.

Required Skills and Qualifications

  • Bachelor's degree from an accredited institution in one of the following healthcare disciplines: Occupational Therapy, Social Work, Registered Nurse, Recreation Therapy, Physiotherapy, or Masters' of Counselling.
  • Registration with respective professional regulatory association required.
  • Minimum 3 years' experience, including clinical practice related to discipline, working in Primary Health Care setting/community-based team/community setting required.
  • Minimum 2 years' experience with Masters' of Counselling required.
  • Facilitation/education skills utilizing adult education, self-management supports, and motivational enhancement principles (both individual and group settings).
  • Experience working with people living with chronic conditions.
  • Experience in partnership development, stakeholder collaboration, and community development.
  • Understanding of population health, health promotion strategies, and behavior change principles.
  • In-depth knowledge of local health authority and community-based services.
  • Demonstrates person-centered/family-focused, diversity relevant approaches to care.
  • Demonstrates innovation in program planning, implementation, and evaluation, and quality improvement.
  • Computer skills (MS Office, email, and internet).
  • Annual CPR-AED certification and annual WHMIS training and certification.
  • Valid driver's license and daily access to a reliable car for travel for work.

Working Conditions

This permanent, full-time position may require evening, weekend, and holiday shifts.
