About the Opportunity

The Continuing Care Coordinator plays a vital role in assessing, planning, and authorizing services for clients referred to Continuing Care. This position requires strong assessment and interviewing skills, as well as the ability to work independently and collaboratively in a busy multidisciplinary environment.

The ideal candidate will have a Bachelor's degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies, and current registration with the applicable Nova Scotia Professional Regulatory body. A minimum of three years' experience in a community or long-term care setting is also required.

The Continuing Care Coordinator will be responsible for:

  • Assessing clients' needs and developing care plans
  • Authorizing services and making referrals to other professional and volunteer resources
  • Working with interprofessional teams to facilitate hospital discharge and return to the community
  • Maintaining ongoing communication with clients, family, caregivers, and healthcare teams

We are seeking a highly skilled and compassionate individual who is passionate about delivering high-quality care to clients and their families. The ideal candidate will have excellent assessment, interviewing, and negotiation skills, as well as strong verbal and written communication skills.

Hours

This is a permanent full-time position, with shifts including days, evenings, weekends, and holidays.
