About the Role

We are seeking a highly skilled and dedicated individual to join our team at Chezranti Community Centre Ltd as a Youth Services Worker. This is a permanent position that requires a strong commitment to providing exceptional support services to our clients.

Key Responsibilities
  • Client Assessment and Support
    • Conduct thorough assessments of clients' needs and develop personalized support plans to address their unique challenges.
    • Provide guidance and support to clients in achieving their goals and objectives.
  • Program Development and Implementation
    • Design and implement rehabilitation programs that cater to the diverse needs of our clients.
    • Collaborate with other service providers to ensure a seamless delivery of services.
  • Case Management and Coordination
    • Manage and coordinate the delivery of services to clients, ensuring that their needs are met in a timely and efficient manner.
    • Develop and maintain relationships with community partners and stakeholders to enhance service delivery.
  • Reporting and Documentation
    • Prepare and submit reports on client progress and program outcomes.
    • Maintain accurate and up-to-date records of client interactions and program activities.
  • Team Collaboration and Support
    • Work collaboratively with other team members to achieve shared goals and objectives.
    • Provide support and guidance to colleagues as needed.
Requirements
  • Education and Experience
    • Bachelor's degree in a relevant field (e.g., social work, psychology, education).
    • 2 years to less than 3 years of experience in a related field.
  • Skills and Abilities
    • Excellent communication and interpersonal skills.
    • Ability to work effectively in a team environment.
    • Strong problem-solving and critical thinking skills.
    • Ability to maintain confidentiality and handle sensitive information.
