About the Role

ChezRanti Community Centre Ltd is seeking a highly motivated and dedicated Support Worker to join our team in a community rehabilitation setting. As a Support Worker, you will play a vital role in supporting individuals with various needs, promoting their independence, and facilitating their successful reintegration into the community.

Key Responsibilities
  • Case Management: Assess clients' needs, develop individualized rehabilitation plans, and implement life skills workshops to promote independence and self-sufficiency.
  • Program Development: Collaborate with colleagues to design and deliver specialized rehabilitation programs, including substance abuse treatment and behavior management programs.
  • Client Support: Provide emotional support, guidance, and advocacy to clients, ensuring their safety and well-being.
  • Community Liaison: Establish and maintain relationships with other social services agencies, healthcare providers, and community organizations to ensure comprehensive support for clients.
  • Program Evaluation: Assist in evaluating the effectiveness of treatment programs, maintaining program statistics, and developing service intervention logic models and outcome measures.
Requirements
  • Education: College/CEGEP diploma or equivalent.
  • Experience: 2 years to less than 3 years of experience in a related field.
  • Language: Fluency in English.
  • Work Hours: 30 to 44 hours per week.
