About the Role

We are seeking a highly skilled and compassionate Support Worker to join our team at ChezRanti Community Centre Ltd. As a Support Worker, you will play a vital role in providing support and guidance to individuals in need, helping them to achieve their goals and improve their overall well-being.

Key Responsibilities
  • Case Management: Assess clients' needs and develop personalized support plans to help them achieve their goals.
  • Rehabilitation Programs: Design and implement rehabilitation programs to help clients overcome challenges and improve their quality of life.
  • Community Outreach: Establish relationships with community partners and stakeholders to ensure seamless support services for clients.
  • Documentation and Reporting: Maintain accurate records and reports to track clients' progress and program effectiveness.
  • Team Collaboration: Work closely with colleagues to ensure a comprehensive and coordinated approach to support services.
  • Client Support: Provide emotional support and guidance to clients, helping them to navigate challenges and achieve their goals.
  • Program Evaluation: Assist in evaluating the effectiveness of support programs and make recommendations for improvement.
Requirements
  • Education: College/CEGEP diploma or equivalent.
  • Experience: 2 years to less than 3 years of experience in a related field.
  • Language: English language proficiency.
  • Work Hours: 30 to 44 hours per week.
