About the Role

We are seeking a highly skilled and dedicated Rehabilitation Service Worker to join our team at ChezRanti Community Centre Ltd. As a key member of our community development team, you will play a vital role in providing support services to individuals and families in need.

Key Responsibilities
  • Case Management: Develop and implement individualized rehabilitation plans to address the unique needs of clients.
  • Service Coordination: Collaborate with community agencies and healthcare providers to ensure seamless delivery of services.
  • Program Development: Design and implement life skills workshops, substance abuse treatment programs, and behavior management programs.
  • Client Support: Provide one-on-one and group counseling sessions, as well as accompany clients to appointments and social outings.
  • Program Evaluation: Assist in evaluating the effectiveness of treatment programs and make recommendations for improvement.
Requirements
  • Education: College/CEGEP diploma in a related field.
  • Experience: 3 years to less than 5 years of experience in a related field.
  • Skills: Excellent communication and interpersonal skills, ability to work independently and as part of a team, and strong problem-solving and critical thinking skills.
Work Environment

The successful candidate will work in a fast-paced environment with tight deadlines and a large workload. They will be required to work independently and as part of a team, and will be expected to maintain accurate records and statistics.

Language

The primary language of work is English.

Work Hours

The successful candidate will work 30 to 44 hours per week.
