About the Position

McMan Youth, Family and Community Services is seeking an experienced Transitional Support Worker to join our Community Based Program team. This role involves supporting youth between the ages of 16 and 22 in the community, promoting their inclusion and connecting them to resources.

Key Responsibilities
  • Meet with youth in the community or in their homes to provide support and guidance
  • Promote community inclusion by engaging in various activities with the youth and connecting them to resources
  • Develop and implement case plans, manage and document youth progress
  • Assist youth in developing life skills and working from a trauma-informed lens
  • Participate in ongoing program development and contribute to a positive team environment
Requirements
  • Completed degree or diploma in a human services field
  • At least 6 months of experience working with youth
  • Police Information Check, including Vulnerable Persons Sector Check, and Children Services Intervention Record Check
  • Emergency First Aid - Level C CPR + AED certification
  • Valid Class 5 driver's license, current drivers abstract, reliable vehicle, and 2 million dollars third-party liability auto insurance
  • Excellent written and verbal communication skills, ability to build relationships with youth and manage crisis
