Vancouver Coastal Health is seeking a Registered Nurse to join the team at Lions Gate Hospital in North Vancouver, BC. As a Nurse Case Manager, you will be responsible for managing a select population of eligible Assisted Living clients. This role requires a comprehensive understanding of case management principles and the ability to work effectively with community resources to plan and coordinate care for clients with complex health needs.
Key Responsibilities
- Manage a caseload of Assisted Living clients and determine their goals and needs
- Assess client situations, work with community resources, and plan for care and support
- Work within a multidisciplinary team context to provide continuity of care across the continuum
- Act as a clinical resource for the interdisciplinary team and provide information on specialized theory and practice related to clients with complex health care needs
Qualifications
- Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM)
- Three (3) years' recent clinical nursing experience in Home Health and case management or an equivalent combination of education, training, and experience
- Local area travel requires the use of a personal vehicle and a valid BC Driver's License
