Salary

The estimated annual salary for this position is $73,584 - $97,221 CAD, depending on experience.

Job Description

We are seeking a highly skilled Admissions Nurse to join our Central Intake team at the Pacific Spirit Community Health Centre in Vancouver. The successful candidate will be responsible for conducting initial client assessments, triage, and referrals to ensure seamless linkages between clients and community resources.

Key Responsibilities

  1. Conduct initial client assessments to determine the level of care required
  2. Triage clients based on urgency and complexity of their needs
  3. Refer clients to appropriate community resources and services
  4. Collaborate with multidisciplinary teams to develop comprehensive care plans
  5. Provide education and consultation to clients and their families
  6. Maintain accurate records and statistics related to client intake and referral data

Requirements

To be successful in this role, you must have:

  • A current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM)
  • Three (3) years recent related experience in the designated clinical program area or an equivalent combination of education, training, and experience
  • Broad knowledge of nursing theory and practice within a client- and family-centred care model
  • Demonstrated ability to provide client-focused, trauma-informed, harm reduction, and culturally safe care
  • Ability to work effectively in a fast-paced environment and prioritize tasks accordingly
