About the Role:
The Canadian Red Cross is seeking a skilled Care Transition Specialist to join our team. As a Care Transition Specialist, you will play a critical role in supporting clients as they transition from hospital to home.
Key Responsibilities:
- Facilitate client transportation home using local transportation services or agency vehicles as needed.
- Support clients in understanding and implementing hospital discharge instructions and develop individualized care plans.
- Complete errands to meet clients' physical needs, such as nutrition, medication, and medical supplies, in accordance with established service plans.
- Plan, arrange, and procure home management duties, such as cooking, cleaning, and laundry, in advance of discharge when identified.
- Facilitate client and caregiver engagement, providing information on client-related issues surrounding transitions from hospital to home.
- Communicate recommendations and support clients, caregivers, and those in the circle of care to address and repair safety concerns.
- Provide companionship to seniors suffering from social isolation.
- Conduct risk analyses of clients' homes and evaluate their risk of falls using standardized risk assessment tools.
- Support clients post-discharge to remove barriers and reduce risk in accessing resources and follow-up appointments.
- Document concerns, observations, and needs of clients in accordance with standards of documentation and communicate with the Program Coordinator or designate.
Requirements:
- Minimum two years of post-secondary education with a concentration in social services, gerontology, therapy assistance, or a related field, and a minimum of three years of job-related experience or an equivalent combination of education and experience.
- Previous knowledge of medical and technical terminology is an asset.
- Fluency in English and French is required.
- Excellent computer skills, including familiarity with Microsoft Office.
- Knowledge of CIMS and other databases is considered an asset.
- Valid driver's license and access to a reliable vehicle.
Working Conditions:
- The work is performed Monday to Friday in a combination of office environments, hospital settings, community settings, agency vehicles, and client homes.
- This job requires some physical bending, stretching, repetitive motions, and possibly climbing, pushing a wheelchair with a beneficiary, and helping them to get into and off the vehicle.
