Job Overview

We are seeking a compassionate and skilled Care Coordinator to join our team at The Alex, a leading provider of healthcare services in Calgary. As a Care Coordinator, you will play a vital role in delivering comprehensive care to individuals experiencing homelessness and poverty.

Key Responsibilities:
  • Conduct initial assessments to determine client eligibility for our services and develop personalized care plans.
  • Collaborate with multidisciplinary teams, including nurses, social workers, and case managers, to coordinate care and ensure seamless service delivery.
  • Provide emotional support and advocacy to clients, connecting them with community resources and services.
  • Work closely with clients to set and achieve goals, promoting independence and self-sufficiency.
  • Document client interactions, progress, and outcomes in a timely and accurate manner.
Requirements:
  • Diploma in Nursing or equivalent experience working with vulnerable populations.
  • Current registration with the College of Licensed Practical Nurses in Alberta (CLPNA).
  • Minimum 2 years of progressive experience in nursing, social work, or a related field.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Able to work independently and collaboratively as part of a dynamic team.
