Job Title: Healthcare Integration Director

Hamilton Family Health Team is seeking an experienced Healthcare Integration Director to join our team. As a key member of our leadership team, you will play a critical role in advancing our strategic plan through the development and implementation of four geographical neighbourhoods across Hamilton.

Key Responsibilities:
  • Develop, implement, and evaluate strategic and operational plans to advance healthcare integration and improve patient outcomes.
  • Build and maintain collaborative relationships with stakeholders across the health system and community to ensure seamless care and improve the patient experience.
  • Lead the development and implementation of innovative and creative approaches for delivering high-quality and efficient healthcare services.
  • Engage with internal stakeholders to develop, operationalize, and sustain the integrated healthcare model.
  • Develop and foster joint accountability through analytical frameworks and performance dashboards to assess progress on our strategic plan and annual business goals.
Requirements:
  • Undergraduate degree in a relevant health-related field required, master's degree preferred.
  • Designation as a Regulated Health Professional and a member in good standing with the relevant professional college or association.
  • Ten years' recent, related experience in progressively responsible leadership positions.
  • Strong leadership experience in healthcare delivery, with a preference for primary health settings.
  • Comprehensive understanding of current challenges in healthcare impacting primary care.
  • Strong knowledge of best practices and benchmarks for advancing integrated care.
