About the Role
This permanent part-time opportunity will involve supporting all inpatient units and programs, ensuring seamless medication management through various functions such as purchasing, inventory management, unit dose drug distribution, and IV admixtures.
Salary Information
The successful candidate can expect a competitive hourly rate of $48.21 - $64.50, with bi-weekly hours amounting to 45 hours, equating to approximately $52,700 - $69,300 per year, depending on experience.
Responsibilities
The key responsibilities include:
- Participating in interdisciplinary rounds and care conferences to ensure safe prescribing decisions are made and adverse events are prevented.
- Conducting medication reviews, developing patient care plans, and communicating these plans to the interprofessional team, patients, and families.
- Ensuring continuity of care by performing medication reconciliation during patient transfers and discharges.
- Preparing and dispensing medications accurately for designated hospital programs.
- Participating in departmental projects and clinical pharmacy programs to enhance medication safety.
Qualifications
To be considered for this role, applicants should possess a current license to practice as a pharmacist from the Ontario College of Pharmacists (Part A), a minimum of 2 years of hospital pharmacy experience, and a strong background in pharmaceutical care.
