Job DescriptionThis is an exciting opportunity for a compassionate and skilled Nurse Practitioner to work in our Bariatric, Surgical Program. You will be responsible for autonomously diagnosing, ordering and interpreting diagnostic tests, prescribing pharmaceuticals, and providing clinical expertise and guidance to patients and families.
You will also have the opportunity to participate in program development and evaluation, analyze statistics, and identify researchable questions. Additionally, you will collaborate with the interdisciplinary team and physicians to provide comprehensive medical care for bariatric patients.
In this role, you will be expected to work independently and collaboratively, communicating effectively with all staff, clients, families, and community agencies. Your strong interpersonal communication skills, problem-solving ability, and critical thinking skills will enable you to establish positive relationships and provide excellent patient care.
Responsibilities- Autonomously diagnose, order and interpret diagnostic tests and prescribe pharmaceuticals within scope of practice as it relates to the Bariatric Program;
- Update policies, procedures and Standards of Care for the Bariatric Program;
- Provide clinical expertise, guidance and education to patients and families;
- Assist in the coordination of activity between the surgical and medical clinics;
- Work collaboratively with the interdisciplinary team and with the physicians;
- Communicate abnormal findings with the team;
- Conduct health assessment and focused physical exam on high-risk patients; identifies normal and abnormal findings through investigation and liaises with PCP as needed and provides appropriate referrals;
- Facilitate the transition of patients post-operatively over to their Primary Care Providers;
- Facilitate team rounds and assist in the provision of comprehensive medical care for the bariatric patient;
- Identifies researchable questions and participates in research initiatives;
- Providing education on matters related to bariatric surgery, weight loss and health promotion;
- Formulates differential diagnoses, orders and/or performs screening and diagnostic investigation based on presenting health concern and treating using evidence based best practices and triaging post-operative for complications warranting ED visit and liaising with surgeons and internist as needed;
- Current registration with the College of Nurses of Ontario as a Registered Nurse in the extended Class (Primary Health Care or Adult) and entitled to practice with no restrictions;
- Member of the Registered Nurses Association of Ontario preferred;
- Minimum 2 years experience as an NP in primary care, long-term care, acute care or continuing care;
- CDE and CBE certification preferred;
- Knowledge of Bariatric, community resources, crisis work and a working knowledge of the Regulated Health Profession Act, Mental Health Act, Health Care Consent Act and Substitute Decisions Act;
- Demonstrated knowledge of the principles of adult education;
- Experienced in Program development and evaluation and analysis of statistics;
- Demonstrated experience and proficiency in the provision of effective clinical consultation and assessment;
- Strong interpersonal communication skills and ability to establish positive relationships with all staff, clients, families, and community agencies;
- Excellent verbal and written presentation skills;
- Excellent problem solving ability and critical thinking skills and able to work independently and in collaboration with interdisciplinary team;
- Excellent computer skills using Office 365;
- Knowledge of Quality Improvement process, data analysis and program development;
- Demonstrated commitment to excellence in patient/family centred care;
- Able to work independently with minimal support.
