Job Summary:We are seeking a highly skilled and compassionate Social Worker II to join our interdisciplinary team. As a Social Worker II, you will work collaboratively with healthcare professionals to provide evidence-based social work services to clients with complex mental health and substance use disorders.
Key Responsibilities:- Provide biopsychosocial assessments, consultations, and treatment plans for clients with severe and complex mental health and substance use disorders.
- Develop and implement individualized treatment plans that address the client's physical, emotional, and social needs.
- Collaborate with healthcare professionals to provide comprehensive care and support to clients and their families.
- Advocate for clients and their families to access necessary resources and services.
- Document client progress and treatment plans in accordance with hospital and legislative standards.
- Participate in interdisciplinary team meetings and contribute to the development of treatment plans.
- Provide education and support to clients and their families on mental health and substance use issues.
- Develop and maintain relationships with community agencies and organizations to access resources and services for clients.
- Master's degree in Social Work from an accredited institution.
- Registration with the Ontario College of Social Workers and Social Service Workers.
- Experience working with individuals and families with complex mental health and substance use disorders.
- Training and experience in evidence-based treatments, such as motivational interviewing and cognitive-behavioral therapy.
- Excellent communication and interpersonal skills.
- Ability to work effectively in an interdisciplinary team environment.
- A competitive salary range of $44.67 to $57.90 per hour.
