Vancouver Coastal Health is seeking an experienced Outreach Worker to join our Overdose Outreach Team in Vancouver's Downtown Eastside. As a key member of our interdisciplinary team, you will provide vital support services to individuals and their families struggling with substance use issues.
Key Responsibilities- Work collaboratively with clients to access opioid agonist therapy and related support services.
- Monitor clients' health conditions and environments, providing guidance and support as needed.
- Arrange and escort clients to appointments, errands, and other activities, promoting independence and self-advocacy.
- Create linkages to community resources and programs, empowering clients to make informed decisions.
- Assist clients in accessing benefits and assistance, providing brief planning and coordination on issues related to substance use and harm reduction.
- Administer medication to clients under the direction of a Registered Nurse, promoting medication adherence and health outcomes.
- Liaise with community resources and healthcare providers, fostering partnerships and collaboration.
- Provide basic health-promoting information and promote life-skills development, empowering clients to take control of their health.
- Perform general clerical duties, attend meetings, and participate in patient care planning and coordination.
Our Overdose Outreach Team is dedicated to providing connections for individuals who have recently experienced opioid overdose or are at high risk for opioid overdose to substance use care and support. We work closely with clients to navigate appropriate services, access Opioid Agonist Therapy, and promote overdose prevention education.
Qualifications- Diploma in Community Social Services or equivalent combination of education and experience.
- One year of related experience working with people who use substances and those suffering from complex mental health issues.
- Valid BC driver's license and vehicle.
- Cardiopulmonary Resuscitation (CPR) certification.
