Community Care Nurse

About the Role:
As a Community Care Nurse with ParaMed, you will have the opportunity to provide compassionate care to patients in their homes. You will work with patients of all ages, delivering care that is tailored to their individual needs.
Key Responsibilities:
- Participate in scheduled visits to assess patient needs and provide care within your scope of practice
- Use appropriate assessment tools and techniques to assess patients' individual needs
- Execute and continuously monitor care plans, making adjustments as required
- Engage patients using a collaborative approach, building rapport and trust
- Maintain and update professional patient records related to patient encounters and plans

Requirements:
- Registration with the CNO as a Registered Practical Nurse in good standing
- Valid driver's license and access to a vehicle
- Experience in delivering community care is an asset
